Utah State Courts - Data Standards
All data submitted to the Utah State Courts is ultimately entered in CORIS, which is the proprietary case management system of the Utah courts. The CORIS system has certain requirements for formatting data, as follows:
  • INDIVIDUAL NAMES
    • LAST NAME WRITTEN WITH TITLE (JR., SR., II, MD, etc...) – The general rule is to leave out any punctuation marks (periods, commas, quotes, etc...). Professional title should not be entered. For example:
        MR. DONALD P. SMITH II => Last name: SMITH, First name: DONALD P II
        DONALD P. SMITH, JR. => Last name: SMITH, First name: DONALD P Jr
        DONALD PETER SMITH, MD => Last name: SMITH, First name: DONALD PETER

    • FOREIGN NAMES –– The general rule is to use the first name listed as the first name, and the third name listed as the last name. (Second name is normally the middle name and entered with the first name.) Hyphens are allowed in CORIS. Note: when you do a name lookup you will need to include the hyphen.
        MIGUEL SANCHEZ HERNANDEZ => Last name: HERNANDEZ, First name: MIGUEL SANCHEZ
        MIGUEL SANCHEZ-HERNANDEZ => Last name: SANCHEZ-HERNANDEZ, First name: MIGUEL
        MIGUEL HERNANDEZ => Last name: HERNANDEZ, First name: MIGUEL

    • Names with a hyphen or a dash (––) Hyphens are allowed in CORIS. Note: when you do a name lookup you will need to include the hyphen.
        CLARE T. SMITH-TODD => Last name: SMITH-TODD, First name: CLARE T
        JO-ELLEN BROWN-JONES => Last name: BROWN-JONES, First name: JO-ELLEN

    • LAST NAME WRITTEN WITH APOSTROPHE (‘) – The general rule is to leave out any quote marks. No spaces should be used to replace the quote mark.
        KATE O'BRIEN => Last name: OBRIEN, First name:KATE

    • LAST NAME WRITTEN WITH IMBEDDED CAPS (McD) – CORIS forces all caps in the last name field so no action is required.
        JOHN McDONALD => Last name: MCDONALD, First name:JOHN
        JAMES MacGREGOR => Last name: MACGREGOR, First name:JAMES
        JANE VAN SAUN => Last name: VAN SAUN, First name:JANE

    • LAST NAME WRITTEN WITH SAINT (ST.) – Saint can be entered as SAINT or ST. The general rule is to use what came in on the filings.
        MR. JOHN SAINT LAURENT => Last name: SAINT LAURENT, First name:JOHN
        JOHN ST. LAURENT => Last name: ST LAURENT, First name:JOHN
        JANE ANN SAINT IVES => Last name: SAINT IVES, First name:JANE ANN
        JANE ANN ST. IVES => Last name: ST IVES, First name:JANE ANN

    • "ALSO KNOWN AS", "DOING BUSINESS AS", TRADING AS" –– Do not make these a part of the party's name. Instead, Enter the party name in the AKA/DBA section:
        FRANK KRAMER DOING BUSINESS AS FRANK KRAMER TRUCKING=> Last name: KRAMER, First name: FRANK, DBA Last name: FRANK KRAMER TRUCKING

  • CORPORATE NAMES
    • SPELL OUT FIRST TWO NAMES

    • ABBREVIATE the following words:
        BOARD OF EDUCATION => BOARD OF ED
        DEPARTMENT => DEPT

    • ABBREVIATE the following, UNLESS they are the FIRST OR SECOND NAMES
        ASSOCIATES => ASSOC
        ASSOCIATION => ASSN
        INTERNATIONAL => INTL
        CORPORATION => CORP
        COMPANY => CO
        LIMITED => LTD

    • DO NOT enter quote marks, apostrophes or periods – Hyphens are allowed in CORIS.
        RENT-A-WRECK => RENT-A-WRECK
        MR. T"R AUTO => MR T R AUTO

    • Drop the word THE whenever possible
        THE FIRST NATIONAL BANK => FIRST NATIONAL BANK
        THE STATE OF UTAH => STATE OF UTAH

    • PDF Format is required for all documents except for orders that require court signature.
    • RTF Format is required for all orders that require court signature.
